New Mexico Awards for Distinguished Service to Animals Nomination Application Instructions These awards are designed to recognize individuals in New Mexico who have dedicated their talents, time and resources for the benefit of providing animals a better way of life. Nominations are open to anyone as long as they are a resident of the State of New Mexico. All nominations, except self-nominations in any category other than Organization of the Year, will receive full consideration. It is possible to nominate more than one person for each of the respective awards. Send separate nomination applications for each nominee Awards will be given to all categories. ALL APPLICATIONS MAILED, SHOULD BE SENT TO: NMWFTA / Nomination P.O. Box 22985-2985 SANTA FE, NM 87505 Phone #: (505) 466-1525 E-mail Address: animalweek@aol.com DEADLINE AND CONDITIONS: Applications must be postmarked no later than August 29 2006. Nominator must explain in an endorsement of 100 words or less why they are nominating said individual/organization for the respective award. All applications postmarked after August 29 2006, not completely filled out, or not including an explanation of nomination 100 word or less , will not receive consideration. WINNERS: The winners will be chosen by an impartial selection committee and then notified by e-mail, letter and/or telephone. Prizes will be awarded at the Awards for Distinguished Service to Animals ceremony to be held(to be announced) as the culminating event of New Mexico’s Week for the Animals. September 23th 2006 on the Santa Fe Plaza, Santa Fe New Mexico.In order for nominees to receive full consideration, the application must be filled out completely.
